Health Assessments

Health assessments are structured questionnaires that help users track and understand their wellbeing over time. Each assessment produces a scored result that feeds into trend analysis and personalised recommendations.

How Assessments Work

  1. Select an assessment — Choose from available questionnaires covering areas such as mental health, physical fitness, nutrition, and sleep quality.
  2. Complete the questionnaire — Answer each question honestly. Responses are scored according to validated frameworks where applicable.
  3. Review your results — View your score, a summary interpretation, and any areas flagged for attention.
  4. Receive recommendations — KORA generates personalised suggestions based on your results, historical trends, and stated goals.

Trend Analysis

Assessments are designed to be taken repeatedly. When multiple results exist for the same assessment type, Surviva builds a longitudinal view that shows:

InsightDescription
Score trajectoryHow your score has changed across sessions
Domain breakdownPer-section scores highlighting strengths and areas for improvement
Milestone markersPoints where significant change occurred, correlated with wellness log entries
Comparison baselinesAnonymous population benchmarks where available

Personalised Recommendations

After each assessment, KORA analyses the results in the context of your full health profile. Recommendations may include lifestyle adjustments, suggested follow-up assessments, or prompts to discuss specific findings with a practitioner. All recommendations pass through the KOS-GUARD-HEALTH-001 guardrail to ensure they remain supportive and non-diagnostic.

Practitioner Integration

Assessment results can be shared with practitioners in your care network. Practitioners receive a structured summary that includes scores, trends, and any areas the user has flagged for discussion. This gives practitioners context before appointments and supports more focused consultations.
